Before signing on please read the following:

How to Join 2011-12

You will need the following forms fully completed for registration for junior member to be accepted:

SLSA Membership Form 2011/12

Parental Consent Form 2011/12

POOL SWIM PROFICIENCY 2011/12

In addition at least one parent must join as an associate member. The associate member will need to complete the following forms:

SLSA Membership Form 2011/12

Blue Card - Volunteer Blue Card Application Form 2011/12

or

Blue Card - Exemption 2011/12
